Academic Forms

Make sure you have the forms and information you need. The chart below has detailed information for the process to complete each form. Some forms are available online (follow the link); while others you will need to pick up at our office in Suite 215. Watch the deadlines and be sure to consult the Academic Calendar for the most up-to-date information.

FORM PROCESS

Add/Drop

  1. Pick up form from Whitman 215. Fill in five-digit class number, course prefix, and three-digit class number.
  2. Obtain signature from course instructor.
  3. Sign and submit form to Whitman 215 for home college signature.
  4. Deliver form to 106 Steele Hall (Registrar's Office).

Grading Options (Pass/Fail)

  1. Pick up form from Whitman 215.
  2. Complete, sign and submit form to Whitman 215 for Home College signature.
  3. Deliver form to 106 Steele Hall (Registrar's Office).

Major Declaration

  1. Meet with the Academic Advisor from your chosen major (or your primary major advisor if you're adding a secondary major).
  2. Complete the major declaration form and leave with advisor for processing. 

Minor Declaration

  1. Print and complete the form.
  2. Obtain signature of the Minor Coordinator.
  3. Sign and submit form to Whitman 215 for processing.

Proposal for Independent Study Courses

  1. Print and complete the form.
  2. Obtain signatures from Faculty Sponsor and Department Chair.
  3. Sign and submit form to Whitman 215 for home college signature.

Internship Proposal/Agreement

  1. Print and complete the form.
  2. Obtain signature from Faculty Sponsor and Department Chair.
  3. Sign and submit form to Whitman 215 for home college signature.

Transfer Credit Petition

  1. Print and complete the form.
  2. Attach course description and/or syllabi.
  3. Sign and submit to Whitman 215 for home college signature.

Leave of Absence/University Withdraw

  1. Meet with an advisor in Whitman 215 to discuss the form.
  2. Complete, sign and submit form to Whitman 215 for home college signature.

Application for Readmission

  1. Pick up form from Whitman 215.
  2. Complete, sign and submit form to Whitman 215 for home college signature.

Petition to Faculty

  1. Print and complete the form, be as specific as possible when describing your request.
  2. Attach any supporting documentation and determine which signatures you need.
  3. Obtain necessary signatures and return to Whitman 215 to be reviewed.
  4. Pick up a copy of your petition to determine whether it has been approved or denied.
